We take the privacy rights of our patients very seriously. This privacy policy sets out how we deal with your personal information.
What personal information do we collect?
In order to treat you, we will ask you to provide certain information. This includes:

• Date of birth
• Home address
• Email address
• Telephone number
• Medical history
• The reasons that you have consulted us
How we collect personal information
All the information we hold is collected directly from you. We may do this at your initial consultation or ask you to fill in a form in advance. Some information may change throughout your treatment so please let us know when we need to make any updates.
How we use personal information
Only the people who actually need to use your data as part of their role in Love My Health will have access to it – this will include your practitioner and also some administration, financial and other support roles if necessary. Some roles may be employees of Love My Health and some maybe external contractors.
We will also use your information to communicate with you about appointments, treatment etc.
Confidentiality
We will not disclose any personal information that we hold about you to any unrelated third party, except where required by law.
If you would like us to work in conjunction with other professionals then we will ask you to sign a document authorising us to pass on the relevant information to them.
How long we keep your personal information
We are required by our insurers to keep data regarding adults for seven years after the final appointment. For children we are required to keep data for seven years after they reach 18 years of age, that is until they are 25 years old.
All personal information will be securely destroyed and digital information erased when we are no longer required to store it.
How your information can be updated or corrected

Under data protection legislation, you have the right to inspect the personal information that we hold about you. You can make a request to do so by contacting us as above and we will respond within 14 working days.
How we store your personal information
Some of your information will be kept on paper records and some may be kept electronically. All data will be kept strictly in accordance with latest GDPR guidelines.
Changes to this policy
This policy may change from time to time. If we make any material changes we will make you aware of them.
