We take the privacy rights of our patients very seriously. This privacy policy sets out how we deal with your personal information, that is, information that could identify or is related to the identity an individual.

What personal information do we collect?

In order to treat you, we will ask you to provide certain information.  This includes:

  • Name
  • Date of birth
  • Home address
  • Email address
  • Telephone number
  • Medical history
  • Details of your doctor

 Over time we will add details of the conditions that you have consulted us for and other information that contributes to the therapeutic approach that has been prescribed or recommended.

How do I collect this personal information?

We collect all the information we need directly from you. Some information will be collected on a medical history form which you may be asked to complete before your first appointment, the rest of the information will be taken during your appointments.

This collection of data is covered under the legal base of “legitimate interests” which is defined within data protection regulation and law.

If there are changes to this policy or how your data will be processed then we will be in touch with you.

How do we use this personal information?

We use your personal information to analyse the health conditions that you have consulted us for and to prescribe an appropriate therapeutic approach.

We will communicate with you by email, other digital methods, by telephone and by post.

Will we share your personal information?

We will not disclose any of your personal information to any unrelated third party, except where required by law or at your request.

If you would like to see another therapist within Love My Health we will ask your permission to share your details and you can decide if you would like details to be shared in full or for some details not to be shared. You can change your mind at any point and we can share the additional details at a later date if you would like us to do that.

How long do we keep your personal information?

We need to keep your information for as long as you continue to consult us and we are required to keep patient information for seven years after their last consultation. In the case of children this will be seven years after their 18th Birthday. 

After seven years your file will be securely destroyed and any digital information will be erased from our computer systems.

How your information can be updated or corrected

To ensure that we have accurate and up-to-date information, please tell us about any changes you believe we should know about.

Under data protection legislation, you have the right to inspect the personal information that we hold about you.  You can make a request to do so by contacting us and we will endeavour to respond within fourteen (14) working days.

How do we store your personal information?

Our patient files maybe kept on paper or electronically, or a mixture of both.

We take steps to protect your personal information against loss, theft, unauthorised access, disclosure, copying or modification. Our paper records are held in a locked cabinet and all electronic information is protected by passwords.

Subject Access Request
If you would like a copy of your records please contact us at christine@lovemyhealth.co.uk or by telephone to 07818 048978. We will reply within a maximum of thirty (30) days.

Data Protection Concerns or Complaints

If you have a concern or complaint about how Love My Health manages confidentiality, sharing of information or the handling of your personal data please address it to christine@lovemyhealth.co.uk. We will reply within a maximum of thirty (30) days.

If your complaint is sensitive in nature and you would prefer to talk to us about it please call 07818 048978. If we cannot take your call, please leave a message letting us know your phone number and a good time to call you back (please give a couple of options as we may be in appointments with clients at certain times).

We will investigate all concerns and complaints as quickly as possible and keep you informed of the progression and outcome by email or telephone.

We are required to record all concerns and complaints and any action taken in response to them. This information is also kept confidentially except when legally required to be given as evidence.

Changes to this policy

This policy is reviewed every two years and we may make changes from time to time. We will notify you if we make any significant changes.

 

Agreement

Continuing with the process of having complementary therapies through Love My Health is taken as agreement to this policy. If you disagree or have any considerations please do let us know.
